Student Medical Insurance Waiver and Enrollment Site
The Spring 2012 Waiver/Enrollment Period: Monday, December 12, 2011 to January 20, 2012.
Incoming Fall 2012 students should check back August 1, 2012 for enrollment and waiver information.
General Requirements for Student Medical Insurance
All Domestic Students billed for 12 or more Credits:
All students billed for 12 or more credits, are required to have insurance. A waiver must be completed and approved to successfully waive the CUA student medical plan and remove the insurance charge. If you believe you have adequate insurance and do not wish to be insured under the CUA student medical plan, an online waiver must be completed. If the online waiver is not completed and approved on or before January 20, 2012 you will be charged $911.00 and you will be enrolled in the CUA student medical insurance plan.
All International Students with F1 or J1 visas:
ALL international students with an F1 or J1 visa, are required to have insurance. This applies to all students with an F1 or J1 visa that are billed for one(1) or more credits. A waiver must be completed and approved to successfully waive the CUA student medical plan and remove the insurance charge. If you believe you have adequate insurance and do not wish to be insured under the CUA student medical plan, an online waiver must be completed and approved. If the online waiver is not completed and approved on or before January 20, 2012, you will be charged $911.00 and you will be enrolled in the CUA student medical insurance plan.
All international students must also present proof of insurance to Natasha Gray, the Student Medical Insurance Administrator. Proof of insurance must be hand delivered to the Office of Human Resources, 170 Leahy Hall.
Domestic Students billed for less than 12 credits and All International Students that do not have F1 or J1 Visas:
Insurance is voluntary. If you wish to enroll in the CUA student medical plan, you must complete an online enrollment application. If an online enrollment application is not completed on or before January 20, 2012 you will not have coverage even if the student medical insurance charge appears on your account. If a charge for the student medical insurance appears on your account, it will automatically be removed unless an enrollment application is completed.
Detailed waiver enrollment information will be e-mailed to the CUA issued e-mail addresses in the upcoming weeks. All students must check their cardinalmail.cua.edu accounts for important information and reminders pertaining to the student medical insurance.
The CUA student medical insurance policy is underwritten by Aetna Student Health. The spring plan is effective from January 1, 2012 - August 13, 2012. The cost of the spring plan is $911.00.

These special categories students are eligible to enroll in the CUA Student Medical Insurance Plan, Optional Major Medical and Dental for themselves. Payment must be remitted to Aetna to complete the Enrollment process. Please follow the link below, click 'Find My School's Plan' and type The Catholic University of America.
