Fall Enrollment has ended.

 

 

 Enrollment Process: 

Students billed on a part-time basis are eligible to purchase the CUA Student Medical Plan on a voluntary basis by enrolling online during active enrollment periods. Notification of the enrollment status will be supplied via email from UHCSR. The student’s account at CUA will be subsequently charged after acceptance of enrollment.

Students please create your online account once you have enrolled. This will enable you to update your information online as needed. Simply go to www.uhcsr.com and create an online account.

New Students entering CUA for the Spring Semester

February 15, 2010

All students who are enrolled by default will have medical card automatically sent to the billing address on file. 

 

Eligible CUA students who have previously waived the CUA Student Medical Insurance Plan, but now have a need to enroll in the CUA Student Medical Insurance Plan may do so by sending an email message to the CUA Student Medical Plan Administrator. In the message the student needs to include name, address, student ID#, date of birth, gender and the reason for the change. The premium rate will be determined by the date prior coverage was lost and the insurance plan period in which that occured. Students who neither enrolled nor waived the CUA Student Medical Insurance prior to the waiver deadline date will be enrolled by default and will receive an insurance card from UHC. The premium for this insurance will be charged to the student's student account. 

 

Enrollment for Spouse or Dependent Coverage

 

Students participating in the CUA Student Medical Insurance Plan may enroll their spouses and/or dependent children in the CUA Student Health Insurance program.  Enrollment for dependents is accomplished by direct application to UHC after the student has enrolled him/herself through the University. For Dependent and Optional Major Medical coverage please go to www.uhcsr.com        

 Please note: payment for these plans is required at the end of the transaction for non-students. 

Enrollment information for Non CUA-Students

If you are a member of House of Studies or Washington Theological Union :

After logging on at UnitedHealthCare: www.Uhcsr.com  select “Find my schools plan” Enter school name and select the “Enroll Now” link.  Please note: payment for these plans is required at the end of the transaction for non-students.
