Spring waiver deadline for New Students Entering Spring Semester is February 15, 2010.

 

 

How to Waive CUA Student Injury and Sickness Insurance Plan

Students must go to www.srstudentcenter.com/cua to waive out of the plan by the deadline date.  Students billed on a part-time basis are eligible to purchase the CUA Student Injury and Sickness   Notification of waiver status will be supplied via email from UHCSR. 

DEFAULT ENROLLMENT

Students who do not enroll or waive online will be enrolled by default into the standard policy and their student accounts charged.

The student medical plan charge is not refundable or transferable.

Waiver Deadlines:

New Students enrolling for the Spring Semester

February 15, 2010

NOTE: If your waiver is rejected, Your policy must be reviewed to ensure that coverage meets CUA Policy requirements.

 

No waiver applications will be reset unless and until policy has been reviewed.

 

 

Students will be enrolled in and charged for the CUA Student Medical Plan unless an online waiver is submitted. Students must be prepared to provide evidence of coverage upon request by Administrator.

 

 

Insurance Policy Review for International Students with Alternative Health Insurance

 

Once international students complete their online waiver, they are instructed to bring a copy of their alternative health insurance policies to CUA Student Medical Insurance Administrator.  In order for international students to complete the waiver process, they must provide a copy of the benefits provided by their alternative health insurance policy to the CUA Student Medical Insurance Plan Administrator for review.  He/She will check to see that the alternative health insurance policy meets all federal government requirements and that both repatriation and evacuation are covered.  A copy of the alternative health insurance policy benefits, written in English, and the corresponding insurance card are required for this review.  If you do not have a copy of your benefits, you should request a copy from the agent who sold you the policy.  Website descriptions of benefits may be used for this review as long as you present your health insurance ID Card.
